Thank you for choosing to refer your patient to Swayne & Partners.

If your patient requires a same-day referral to our emergency team, please do not use this form. Instead, please call our team on 01284 70144 and they will be happy to arrange this.

Which discipline would you like to refer to?

If you would like us to take on the management of a sick patient, please refer them to Internal Medicine. If you would like us to perform x-rays, scans, or endoscopy, and then send the patient back to you for ongoing management, please refer the patient to Imaging.

If you would like to refer a patient for a problem type not listed here, please do call the team to discuss. We take other referrals on an ad hoc basis, as long as the owner in is informed in advance.

        About You

        About Your Pet

By law, pets receiving prescription medication must be seen by a vet at least every six months for a check-up and review. Some pets with more serious conditions may benefit from reviews more frequently.  If your pet has not had a review within this time, we may ask you and your pet to come in for an appointment before we can dispense further medication.

For prescription flea, tick & worm treatment, your pet only needs to be seen by a vet every twelve months. However, a recent change in the law means that this treatment must be specifically authorised by a vet on your pet's medical record at the time they see your pet. If you have not discussed flea, tick & worm treatment recently with a vet, your pet may need to come in for an additional check-up and review.

It may take up to five working days for your pet's prescription to be ready. If you need their medication sooner than this, please call the practice and speak to one of our team.
